POSITION SUMMARY:

Under the direction of the Clinic Coordinator, this position is established to provide ambulatory care nursing services to patients seen in our clinic.  The Family Nurse Practitioner will promote continuity of care, provide patient education, and provide health maintenance to clinic patients. Clinical supervision will be provided by the Chief Medical Director.

 

ESSENTIAL FUNCTIONS:

  1. Provides comprehensive nursing, preventative, and therapeutic health care service to patients and family members in a community based primary care facility as the primary health resources at the facility.
  2. Interview and examine patients using the Electronic Health Record (hereinafter termed the “EHR”), review past medical histories and request diagnostic tests and necessary to obtain all possible information relevant to each patient’s case and care.
  3. Make preliminary diagnosis, direct, prescribe and provide treatment, or arrange for specialized care through referrals of patients examined, if determined appropriate.  For those patients not requiring referral, to provide definitive management of their cases and care.
  4. Provide urgent care as required.
  5. Modify treatment regimens as required, and review patient’s medications, diet, therapy course, adherence to applicable management plan. Obtain patients observation of treatment course, and to ensure patient is instructed and counseled on the patient’s condition and medical regimen.
  6. Complete all reports required by the Bay Mills Health Clinic and participate in program committees and quality assurance activities.
  7. Provide assistance to other medical and administrative staff in areas regarding technical expertise in medical areas.
  8. Must demonstrate positive communication through friendly, pleasant, and professional verbal skills.  Demonstrates the ability to meet the needs of the patient in a caring, competent manner.  Accurately prioritize information and refers to appropriate personnel.  Maintains confidentiality with respect to any and all information learned through performing this role.
  9. Initiates evidence-based care planning with the patient/family and care team.
  10. Evaluates patient’s response to care through observation and motivational interviewing techniques.
  11. Makes/tracks referrals to community-based resources.
  12. Manages care of patients through collaboration with members of the health care team and other health professionals in order to make decisions about treatment and assist the patient in attaining health goals.
  13. Monitors evidence-based clinical indicators to assess progress toward better health and shares these with the patient/family.
  14. Must attend all mandatory or departmental training offered by the Bay Mills Human Resource Department and/or Bay Mills Health Center.
  15. Other duties may be assigned within the scope and complexity of this position’s essential functions.

 

PHYSICAL REQUIREMENTS:

While performing the duties of this job, the employee is regularly required to sit, stand, and walk.  The employee’s work is completed from the medical clinic area and involves direct, daily contact with sick patients.  The employee may come in contact with communicable diseases and appropriate precautions must be taken to prevent the spread of these illnesses or diseases.  Some diseases have the potential to be life threatening as in the case of some blood borne pathogens and specific protective measures must be followed at all times.  The employee may be required to lift patients, or other strenuous physical activity when rendering first aid or while providing patient care.  The ability to communicate through calm, private, non-threatening communication is essential. The employee completes their work under normal office conditions, temperatures, and is environmentally controlled the majority of the time.  Potential for exposure to blood or body fluids or other infectious diseases is present during patient and family contacts. 

 

POSITION REQUIREMENTS:

  1. Certification as a Family Nurse Practitioner with the American Nurses Credentialing Center or American Association of Nurse Practitioners required.
  2. Michigan license to practice as an Advance Practice Registered Nurse required.
  3. Working knowledge of computers. 
  4. Experience in clinical filing and record keeping.
  5. Experience in working with and/or knowledge of Native American culture, preferred.
  6. Familiarity with privacy and confidentiality regulations and implementation of.
  7. Must have excellent past work history and excellent attendance record as demonstrated through employment references.
  8. Must posses a personable, client friendly attitude along with organizational ability to handle multiple tasks simultaneously.
  9. Must successfully pass all background checks.
  10. To perform this position successfully, and individual must be able to satisfactorily perform each function listed under the essential functions and physical demands categories of this position description.
